What it means
The gateway checks every message against your app's grants, and every story.context snapshot of a known story against your workspace's story-ownership setting. When one of those lookups fails, the message is refused before anything is stored or delivered, so nothing was accepted.
How to fix it
Retry the same message with the same message_id; it is safe to resend. If it keeps happening, ask RND.
