Twelve-field SkillWarning per Skills Integration Spec v2 §4.4.2, ratified as the normative contract by decision 21. Severities are the permanent lower-case carve-out (decision 13). timestamp lives on the ENVELOPE (decision 18). instance_ref is RETIRED (decisions 3 and 21) and hard-rejected — outputs scope to their firing level.
